Please provide:

1. Securities Account Opening Form
2. overlay-terms-for-joint-custody-account-eng.pdf
3. Identity Proof of Guardian
4. Resident Proof (within 3 months) of Guardian
5. Birth certificate of whom has not attained the full age
   (If the two parties are not immediate relatives, proof of the relationship between the minor/beneficiary and the guardian/authorized person must be submitted at the same time)
6. Initial Deposit Proof(not fewer than HKD 40,000)
